Coaching Agreement 1
Thank you for choosing Mrs. Alisa Sane practice! We are committed to providing quality care and service to all our clients. The following is our coaching agreement, which we require that you read and agree to prior to any service. The time of the coaching visits will be determined by the Coach and the client based on a mutually agreed upon time. The Coach will initiate all scheduled calls via our telehealth app (Zoom) or telephonically. Initially, you will have a 15-30 min free discovery call/meeting, followed by a 45-60 min initial coaching assessment and re-occurring weekly/bi-weekly visits. If you need to reschedule any session, please let your coach know 24 hours in advance. It is asked for a commitment of at least three months. After that time, you and your coach will reevaluate your progress and, most likely, transition to bi-weekly if attending weekly, or if attending bi-weekly to monthly we can determine these options at the appropriate time. Preparation After your initial session, you are asked to come to the coaching sessions prepared with an idea of what you’d like to achieve in each session as well as successes and challenges in the week leading up to your visit. Coach’s Expectations: • Coaches will be punctual, keeping scheduled appointments, and ethically maintain confidentiality. • Coaches will bring their full self to visits and listen with full attention providing a safe, supportive environment to increase trust and rapport. • Coaches will value the client as the driver of their health and well-being by respecting client’s autonomy and support their goals. Client Waiver: You, understand that you are working with a Certified Life Coach/Certified Health Coach. The coach cannot be held liable for suggestions made within the visits, via app and resources or education that are provided during our work together. Financial Responsibility • Fees are payable when services are rendered, at the time of service. We accept, credit card, pay pal, venmo, cash, cash app. We are excited to have you participate in our practice. Our Life Coaching program are dedicated to your well-being and total wellness. We do offer a sliding scale of additional 15 % off which will be looked at on an individual basis. You “the client” can choose during the free discovery call to discuss whether this will help you to make a decision to move forward through next step. Charges: • Free Discovery Call 15-30 mins-Zero • Life Coaching Session via Zoom -$125-150 •or mobile if local.
Session telephone-$125-150 .Sliding scale is 15% off original fee per session.
Health coaching $50.00 via Zoom/mobile. If local. Relationship Coaching 125.00-150.00 via Zoom or telephone. Plus mobile. if local.
Definition of a “No-Show” Appointment We define a No-Show appointment as any scheduled appointment in which the patient either: Does not arrive to the appointment Cancels with less than 24 hours’ notice Arrives more than 10 minutes late and is consequently unable to be seen Impact of a No Show Appointment No-Show appointments have an impact in our business we provide to our clients: -Another client could have had that slot- -Is disrespect to my practice time- To Avoid Getting a No Show Charge A) Confirm your appointment B) Arrive 5-10 minutes early C) Give 24 hours’ notice to reschedule an appointment July 8, 2021 Consequences of No Show Appointments 1st No-Show- Email sent to patient reminding them of the policy 2nd No-Show-Letter sent and patient billed appropriately: (see below) You agreed that it is your responsibility to notify Alisa A. Sane of the need to cancel a visit 24 hours in advance of the scheduled call/visit. If you are 10 or more minutes late for a schedule visit, the visit will be cancelled and considered a no-show. Coach will attempt in good faith to reschedule missed
